This paper copy of an Interpreter Request Form is a formal document to prove you requested interpreting services.

VRI Refusal Form
Have you ever been forced to use VRI, only to be met with a blurry screen, slow internet, and a no good interpreter?
Hearing people do not know that VRI doesnt work. VRI companies sell them VRI services promising them it works, but it does not. They use it for spoken languages and think it works for ASL. You have the right to request an in person ASL interpreter so you have full access to your medical treatment under Title II of the Americans with Disabilities Act.
You can print this document to formally request an in-person ASL interpreter if VRI doesn not work for you.
